A bonafide certificate certifies that a person is genuinely (bona fide) a student of an institution or an employee of an organisation at the time of issue. Banks ask for it when opening student accounts or processing education loans, visa and passport offices ask for it as status proof, and transport authorities ask for it for concession passes.
It is issued by the institution or employer on letterhead, naming the person, their class/course or designation, the period of association, and — importantly — the purpose for which it is issued. The format below covers both the student and employee variants; strike out whichever does not apply.

How is a bonafide certificate different from an ID card copy?

Should salary be mentioned in an employee bonafide?
Only if the receiving authority needs it (some visa and loan processes do). Otherwise leave it out — a bonafide is status proof, and salary is better evidenced by pay slips issued through payroll.
